For library-owned books, scores, media, etc that will be placed on the reserve shelf, indicate “See item.” The system will supply appropriate information.
For sources without any clear responsibility (web pages, some online reference articles, etc) indicate “anon.”
For all else (online, Digital Orpheus, online tracks from vendors), indicate the Author (for books and articles), Composer (for Scores, Media), or Performer (for Audio) as you want it to appear in the reserve list. Note: you may use composer or performer for audio tracks depending on which is more important.

For Naxos and some other online resources copy the “static URL” on the page.
For many other online sources, simply copy the URL from your browser.
For personal items simply type “Personal” including keys, books, scores, media, etc.
For CD, DVD, LP, DAT and other tracked media indicate track no. (i.e. track 6; side B track 2)
For Cassette, VHS, Reel or other non-tracked media indicate timing (i.e. 1:27-6:36)
For scores, books, articles and paginated material indicate page nos. (i.e. pp. 57-72)
If an item is unavailable, we might substitute an alternative performance (of audio or video) or edition (of a book or score). Check this box if you want only this exact performance or edition. Leave blank to allow library staff to substitute alternative.
