Faculty who are teaching multiple sections of the same course or are teaching multiple courses they wish to combine can make a cross-list request with the form below. Make a note of the course IDs of the courses you’d like combined and list them in the contact form.
Course IDs can be found in the upper left corner of your Canvas course before any changes are made to Course Details under Settings.
If you have made changes to one section please identify which section that is otherwise the changes you have made will not appear in the new cross-listed section.

MATH430_0002_FA19
